Disclosure & Agreement
Please be advised that all clients are required to read entirely and agree to this Disclosure and Agreement before beginning services with Share-Consulting Human Services. We have made it as simple and straightforward as possible to provide full transparency and good faith in the provision of our services.
1. CONFIDENIALITY POLICY:
In accordance with Share-Consulting Human Services' policies and procedures, all client information will be kept in strict confidence and will not be disclosed under any circumstances, with the exception of information as required by law or a client's revelation to cause harm to self or others. In such cases the client will be advised to contact the local police or emergency services for immediate help (Hotline Numbers are listed on our Contact Page).
2. CLAIMS TO RECOVERY POLICY:
Share-Consulting Human Services does not make any claims to recovery from severe and serious mental health illnesses through its practices. However, strong evidence indicates that extreme stress is at an all-time high in society, contributing to non-performance and mental health breakdown. In these conditions, the use of Professional Services in: Counseling, Psychotherapy and Performance Improvement, has shown significant success in helping to alleviate and mitigate the effects of psychological stress on the human condition.
3. ONLINE AND PHONE SAFETY POLICY:
Online and Phone sessions carry particular requirements for keeping clients' information safe. We require clients to use secure personal devices and web/internet connections for their sessions, as we cannot guarantee breaches to online privacy. On the part of Share-Consulting Human Services, we use Avast as our preferred security platform for our Website. All Therapists are required to use secure devices for all Online Sessions, as well as internet security. We use Zoom and Google Meet as our preferred platforms for added security. Phone and WhatsApp, calls may be used for filler sessions, as long as clients feel confident that safety and security are not breached.
We do not sell or share your personal information with any third party person or group. All information entered into our online databases is Managed by our website host: Wix (You may access Wix's Policies for details on Customer Privacy). Your information collected via our forms are used solely in-house for the purpose of client case consultations and research. No identifying personal information (name, address, email, phone number, other), will be shared without the expressed informed and written consent of the client.
5. PERSONAL SAFETY POLICY:
Clients agree to practice safety protocols for themselves and others while engaging in services with Share-Consulting Human Services and our Team of Consultants. This includes Physical Safety, No Threatening to the Safety of self and others, No Illegal or Unethical Conduct.
6. GOOD-FAITH INDEMNIFICATION POLICY:
In the spirit of Unintentional Harm and Good-faith by and between all parties, including clients and organizations, involved with Share-Consulting Human Services and its Team of Consultants, both Share-Consulting Human Services' Team of Consultants and all parties, including Clients and Organizations, agree to indemnify each other of all wrong-doing, known and unknown, including all legal fees, in the past, now and forever. However, this does not prevent any Legal, Voluntary, Mutually-agreeable, Commonsense and Reasonable Correction, Apology and/or Compensation, by and between all parties, including Clients and Organizations.
7. POLICY & PROCEDURE UPDATES:
Share-Consulting Human Services will do its very best to inform of significant updates to its Policies and Procedures, with every consultation. However, Policies and Procedures can be updated at any time without any notice and will supersede all previously existing policies up to that time.
8. AGREEMENT & ACCEPTANCE: Your continuation of services with Share-Consulting Human Services and its Consultants, indicates your agreement to these policies.
